At Grace Remote Support Services, our Specialist Support Coordination service is designed for NDIS participants living with complex needs, particularly in remote and Indigenous towns and communities. We walk alongside you and your support network to help overcome barriers, manage challenges, and build strong, connected supports that respect your culture, community, and individual goals.
What We Provide
Our Specialist Support Coordinator Team offer expert help to:

Navigate challenging life circumstances, such as involvement with justice, unstable housing, or mental health concerns.

Coordinate supports across health, disability, housing, justice, and community services.

Develop and manage risk plans, crisis responses, and service agreements
-
Support guardians, families, and communities to work together in decision-making.

Build capacity for independence while ensuring you’re safe, heard, and supported.
Who This Is For?
This level of support is best suited for NDIS participants who:

Are experiencing frequent crises, risk of homelessness, or complex mental health conditions.

Live in remote locations with limited access to services.

Require assistance to engage services in a culturally safe, trauma-informed way.

Have significant challenges coordinating support from different systems (e.g. hospitals, police, child protection).
